Bring extra copies and be sure your money order is in order. They will reject your application for the slightest error. In my case I was told – by the BLS yesterday – that the total fee was $151. Today I was informed that the total fee is $153 so I had to go find a two dollar money order and pay the BLS $75.00 for the privilege or make another appointment.
Here’s my checklist based on today.

- Address is: 5455 Wilshire Blvd 10th Floor Suite 11. Any other address on Google or Maps is incorrect. You must be on time but may end up waiting an hour+. Door will be locked. Knock and wait. Present your Apt Confirmation and drivers license at the door. Do not wait in the lobby. Wait outside the door because if you wait in the lobby you will be forgotten. If you’re a minute late they will cancel your appointment. There is on street parking on the side streets.

3 months most recent bank statements plus one from December of previous year. Bank letter providing annual average, length is acct held, acct in good standing. Social Security Statement. Last year’s taxes – both in English and Spanish.
Visa Fee – $153.00 – money order or certified bank check
