-application form (photo glued on)
-ex-01
-form 790-2 copies
-2024 tax return (1040 tax form) you cannot download this from the IRS website. I downloaded from TurboTax or wherever you filed.
-3 months of bank statements (February 2026, January 2026 and December 2025) plus December 2024 (they want full December so also include your January following) in my case I had to include January 2025 which had the rest of my December 2024.
–letter of intent (they didn’t ask for my commitment to not work because it was stated in my letter of intent) I provided it anyway
-copy of your ID
-copy of your passport
-physical passport
-FBI report-original plus copy, official translation and apostille
-medical certificate-2 copies
-health insurance form
-money order (I got it from USPS but the BLS main site says they don’t accept it anymore. They still took it. I’d get it from your bank just to be extra cautious)

Things I was not asked for
-resignation letter
-3 month accommodation

Thank you to everyone who provided details on their experience. Now we wait for approval! They told me LA has been approving quicker than usual so who knows. My entry date is July .
