“Does the form not need to be presented in Spanish at the consulate though?”

“Yes, it needs to be the original Spanish form, but not presented at the consulate/BLS. It is downloaded, filled out, and then uploaded, along with your ID etc., on the INSS registration website. Once you receive an email that you are (pre)registered, print it out and take it as proof.”

“If it’s of use to anyone, here is a good quality jpeg translation, Page 1 and 2 on one A4 landscape page. Please be aware this has been done by Google Translate, so I am not responsible for any errors.”
